The report we stopped using

A successful handover report could conceal an unsuccessful inheritance. In 2009, the institution stopped calling that continuity.

The service did not stop. This statement is retained. Its use as the complete finding is withdrawn.

01 / The Seamless study

S-06, informally called Seamless, began within MNEME in 2006. It investigated service handovers in a controlled bench system. Its summary format emphasized uninterrupted output during replacement. Detailed logs retained the commitments presented to the successor, but the summary did not distinguish a commitment accepted by the new system from one merely copied into its history.

02 / The objection

A methods review found that a recipient could continue the service while declining a prior obligation. The summary still classified the handover as successful. No live public service was involved. The fault was in the scope of the reported success: operational availability had been allowed to stand for a stronger claim about responsibility.

03 / Retirement

The S-06 study was closed on 21 October 2009. Its reporting format was withdrawn. The results were retained as measurements of availability, with the broader continuity claim removed. MNEME continued under explicit acceptance criteria for inherited obligations. S-06 was a study within the program, not a sixth program whose existence has been suppressed.

04 / A habit that survived

Subsequent review forms separate service continued, obligation accepted, and authority established. The fields can carry different answers. Older accounts are not silently relabeled with the improved terminology. A later reader must be able to see what the institution once thought was sufficient, and why it ceased to be sufficient.
